1 Hour Assessment: $150.00   1 Hour Individual/Family: $90.00  

LifeSTAR Phase I Group: $440.00  LifeSTAR Phase II or III Group: $45.00 per session   

LifeSTAR Phase I Workbooks: $100.00   LifeSTAR Phase II Workbooks: $80.00

Returned Check Fee: $25.00   Missed Session Fee: Full Charge  

Insurance

I am currently not taking any form of insurance. Unless agreed upon differently I will provide you with a copy of your receipt on a monthly basis, which you can then submit to your insurance company for reimbursement, if you so choose. Not all issues/conditions/problems dealt with in psychotherapy are reimbursed by insurance companies. It is your responsibility to verify the specifics of your coverage.
